WELCOME TO THE VRT FAMILY!
We are so glad you are joining us.
Before you begin your engagement with VR Theatrical, there are two forms that we need you to complete.
IMPORTANT
Please complete BOTH forms.
The Employee Onboarding Form provides us with the information required for your contracting, payroll and administrative records, while the Marketing Materials Form provides us with the information and assets required for publicity and communications.
Please complete both forms as soon as possible so that we can make sure everything is ready for your engagement.

1. EMPLOYEE ONBOARDING FORM
Required for all new employees/contractors.
Completing this form provides us with the information we need to prepare your contract, payroll profile and employee records before your first day.
Please ensure that all information is accurate and up to date.
2. MARKETING MATERIALS FORM
Required for all cast and creative team members
These materials will form your publicity profile with Maynardville Theatre and/or VR Theatrical and may be used in production programmes, on websites, in media releases, on social media and in other approved publicity relating to your engagement with us.
Please ensure that:

Your credit name is written exactly as you would like it to appear publicly.
